Out-of-Class Testing Appointment Request Form

Submit test requests at least one week in advance (2 business day minimum notice required)
Students must have prior approval from LAP Specialist/Counselor
The Authorization for Testing Accommodations form must be on file with LAP
Tests must be taken on the same campus as the class
